Does anyone know a website that allows you to print legal forms for free, specifically Advance Directives?

I have heard of a legal form that allows terminally ill patients to basically run down a list of critical healthcare scenarios and options and select for each whether or not they would want that procedure if they were unable to speak for themselves, more situation specific than living will physicians directives (general life sustaining terms). She’s tired of being harrassed by the doctors about how to care for her–well she’s just plain tired, she’s dying and I think this would help. The hospital knows what I’m talking about, but they don’t have these forms only living will forms. I know I’m not crazy. I think it is called an Advance Directive. I want my sister to be able to more specifically tell her doctors what she wants from them in these last few months so they can leave her alone and let her rest. Can anyone help?

It is called an Advanced Health Care Directive, it is a living will and healthcare power of attorney in one. Try www.findlaw.com if not then you probably can’t get for free without buying a program. I reccommend buying a family law program if you don’t want an attorney. It is always best to at least get the advise of an attorney before making legal decisions or filling out legal paperwork so you know what your legal right and responsibilities are.
