Living Will Information Sheet
Help me please?
Ok, so yesterday I ordered a cap from Los Angeles and I live in the UK. They sent me an email today saying the following:
We were unable to confirm your information with the bank. Pleasefax 001-(323-234-4965) or e-mail us a billing statement issued to you by your bank which includes your name, address, and the cardnumber that you used to place this order. When sending via fax,please reference your order number on the fax cover sheet. This information will be used to verify that we are shipping to thecardholder’s correct billing address. We will not be able to clearyour order for shipment until this has been completed. If you arehaving trouble producing this additional verification or have anyquestions regarding our policy, do not hesitate to contact us.
Ok so how do I get a billing statement? And aren’t I going to have to scan it to email it?
Is there any information on the billing statement which I should take out?

Where did you order it from? Was it a known, reputable company? My first thought was that it sounds a little fishy to me, but I could be wrong. I would check into the company before you send them anymore personal information… unless of course it’s someone you’re familiar with.
Yes, you would have to scan it to email it, unless you have online banking… but again, make sure you know who you’re sending it to.
