How do I make a living will document?

I’m 20 yrs old, and I really want to make a living will for myself in case anything will happen.

How do I make one? I read books on making a living will and they have an example of what a living will look like. except it has blanks so I can fill in my names and what I want ( DNR or Full code except cardiac arrest..etc.)

Do I make a copy of this sheet of example? then fill it out and carry it with me? I’m confused..I thought people need some kind of special document…not just making copies..I know who’s my proxy is..The book suggests me to make a copy of these sheets of paper(my living will) and keep them someonewhere safe.

Do I need to work with other healh professions/people to tell them what I want in my living will? like the U.s. Living will Registry? www.uslivingwillregistry.org

I live in California.

Any chaplain within any hospital can help you make these arrangments for free. You can also pay a lawyer (usually around 100$ to do the same thing. Or, you can print the document and do so yourself. However, you must sign infront of two witnesses and a notary. They have to be notorized. Your choosen poa needs to have the ORGININAL copy. You need to give a copy to anyone named on the directive, as well as ALL doctors. You can make as many copies as you want, but the original needs to stay with your proxy and be notarized.

DNR in my state atleast has to be filled out be an actual doc. You MUST keep this with you and be able to show for it to be effective. In an emergency, they are required by law to resucitate unless they can actually see the document.
